The Office of the Registrar has compiled a list of important forms and resources students will need throughout their time at Whittier. Contact the Office of the Registrar for information not provided here.
Forms for Undergraduate Students
Academic Review Appeal
: Appeal academic suspension and full probation sanctions.
Academic Overload Petition
: Petition to take more than 16 units during a regular semester or summer term.
Add/Drop Form
: To be used to add or drop classes after the deadline for open registration
Application for Directed Study
: Complete this form if you are requesting to take a directed study course.
Apply for Graduation
: This process is completed on MyWhittier. Visit the
Commencement Checklist
page for step-by-step instructions.
Biographical/Name Change
: To request a biographical/name change on academic records.
Certification Request for VA Benefits
: Complete every semester
Change of Advisor
: Required to change from one advisor to another or to add a second advisor.
Concurrent Enrollment Form
: Required for students who intend to take courses outside Whittier College for the purpose of receiving transfer credit.
Course Override
: The Course Override Form is required when a student wishes to override a course prerequisite
Course Time Conflict Form
: Complete if you are registering for two courses that conflict in time.
Credit/No Credit Option
Commencement Agreement
: To participate in commencement while being short of completing graduation requirements.

: To drop classes.
Enrollment Verification Request:
The
National Student Clearinghouse
provide lenders, insurance companies, employers, and other third parties instant enrollment and degree verifications. Current students who need to verify current term enrollment, should access the National Student Clearinghouse Student Self-Service link from My.Whittier.edu. Select the "My Info" tab, then click the "Enrollment Verification" link in the "Student Registration Tools" menu. There is no charge for this service.
FERPA Release of Records
: Use
Poet Proxy Access
to grant a parent or another person access to specific
My.Whittier.edu
pages the student authorizes. A Poet Proxy Access account must be established with a secure pass phrase before Registrar's Office staff will discuss a student's confidential information with a third party over the telephone.
FERPA Request for Confidentiality
: To prevent the College from sharing "directory information" with an unauthorized third party.
FERPA Request to Inspect/Review Records
: To review a student's official (paper) academic record.
Incomplete Extension Request
: To request an extension for an approved incomplete grade.
Incomplete Grade Application
: To apply for a grade of Incomplete with an instructor.
Independent Study:
Complete this form if you are requesting to take an independent study course.
Instructor Permission Form
: Complete this form if enrollment in a course requires instructor approval.
Non-Degree Application - High School Dual Enrollment
: Please review the
Dual Enrollment policy
before applying. If you are currently in high school and want to start earning college credit prior to graduation.
Non-Degree Application - Graduate
: If you already have a bachelor's degree and are not seeking another degree but want to take courses at Whittier College.
Non-Degree Application - Undergraduate
: If you are not a degree-seeking student but want to take courses at Whittier College.
